Red light therapy (RLT), also called photobiomodulation, uses specific wavelengths of red and near‑infrared light to interact with tissues and influence cellular function. It’s noninvasive, increasingly popular, and supported by a growing body of research for certain conditions. This post explains how it works, what applications have the strongest evidence, typical protocols used in studies, and where the evidence is still limited.

What Is Red Light Therapy?
RLT uses narrow bands of red and near‑infrared light to target light‑sensitive molecules inside cells. Common wavelength ranges:

  • Red light: 620–780 nm
  • Near‑infrared (NIR): 790–1400 nm

These wavelengths penetrate skin and underlying tissue to different depths and are absorbed primarily by molecules in mitochondria. By interacting with these molecules, RLT can alter how cells produce energy (ATP), respond to stress, and initiate repair processes.

How It Works
A key mitochondrial enzyme involved is cytochrome c oxidase, which can absorb red/NIR light. When stimulated:

  • Cells can produce energy more efficiently (increased ATP).
  • Oxidative stress handling may improve.
  • Inflammatory signaling can be modified.
  • Repair and regeneration signals may be activated.

Clinical Applications with Stronger Evidence

Acne

  • Mechanism: Blue light (~415 nm) activates porphyrins from Cutibacterium acnes to reduce bacterial activity; red light (~633–660 nm) penetrates deeper and helps reduce inflammation.
  • Typical study protocol: 2–3 sessions per week for 4–8 weeks, 10–20 minutes per session; many studies use combined blue + red or alternating sessions.
  • Evidence: Human trials show reductions in inflammatory and noninflammatory lesions. Best for mild to moderate acne; not usually first‑line for severe nodulocystic acne.

Hair Loss (Androgenetic Alopecia)

  • Wavelengths studied: ~630–690 nm (many consumer devices use ~655 nm).
  • Goal: Stimulate follicles and encourage transition from telogen (resting) to anagen (growth).
  • Typical study protocol: 15–25 minutes per session, about 3 times per week (or every other day) for at least 16–24 weeks; devices usually helmets, caps, or combs placed close to the scalp.
  • Evidence: Randomized trials report increases in hair density with consistent, long‑term use. Works best as part of a comprehensive treatment plan.

Wound Healing and Tissue Repair

  • Rationale: RLT can support fibroblast activity, collagen production, angiogenesis, and other repair mechanisms.
  • Wavelengths used: red (630–660 nm) or NIR (800–904 nm).
  • Typical regimen: repeated treatments 2–3 times per week over several weeks.
  • Evidence: Promising results in chronic wounds, diabetic ulcers, and some post‑surgical applications.

Pain Relief and Inflammation

  • Best evidence: selected local conditions such as chronic neck pain, some knee osteoarthritis protocols, and other superficial musculoskeletal problems.
  • Example settings used in studies: 785–860 nm delivering ~4–8 J per point, or 904 nm delivering ~1–3 J per point, typically 2–3 sessions per week.
  • Note: RLT can be a useful adjunct for localized pain but is not a universal solution for deep or systemic inflammation.

Where Evidence Is Weaker
Areas with limited or inconsistent human data include:

  • Fat loss and body contouring
  • General athletic performance or recovery (mixed results)
  • Broad metabolic health or whole‑body “anti‑aging” claims

What Determines Results
Success depends on several technical factors:

  • Wavelength: different targets and depth of penetration.
  • Dose: irradiance (mW/cm2) and total energy delivered (J/cm2) matter more than time alone.
  • Device quality and design: clinical vs. consumer devices can differ substantially.
  • Treatment consistency and duration: many conditions require weeks to months of regular sessions.

Safety and Practical Advice

  • Select devices with known specifications (wavelength and irradiance).
  • Follow dosing guidance from manufacturers or clinicians; more power isn’t always better.
  • Use eye protection when recommended, especially for high‑intensity or NIR devices.
  • Consult a healthcare professional if pregnant, on photosensitizing medications, or treating active cancer sites.
  • Track progress with photos or symptom logs to evaluate effectiveness over time.
